Transitional Care Unit
Service/Responsibilities
Transitional Care is responsible for those patients who have had a 3-day hospital stay who require further hospitalization for a skilled need, such as skilled nursing, physical therapy, occupational therapy, speech therapy, or a combination of skilled needs.
Staffing
We are a 30 bed unit with an average daily census of 24 residents. The residents are divided equally among the staff that is scheduled.
Training/Education
Owensboro Health offers many classes that benefit the staff. Residents are admitted with medical, surgical, neurological and cardiac diagnoses, so many Owensboro Health offerings apply to nurses working in our unit.
Continuous Quality Improvement
Transitional Care participates in Fall team, Skin team, and other process improvement teams. We monitor eleven indicators in compliance with state regulations. Team members have a weekly conference to discuss each resident's care, progress, medications, and discharge plans. The team members include: the medical director, clinical supervisor, case manager, MDS coordinator, therapies, dietitian, chaplain, pharmacist, manager, program director and activities coordinator. The family and resident are also encouraged to attend the weekly conference.
Transitional Care has been found deficiency-free in the past three annual state surveys.
