Owensboro Health Transitional Care Center Named "Best of Kentucky in Nursing & Rehabilitation"
Owensboro Health’s Transitional Care Center is a source of excellence in nursing and rehabilitation. Its unwavering commitment to delivering safe, patient-centered care has not gone unnoticed, as the center was recently honored with the respected “The Best of Kentucky—Nursing and Rehabilitation” award.
"OH TCC provides excellent care by providing empathic, compassionate, holistic services to residents, their families and caregivers," said Jenna Hack, Manager of Transitional Care Center of Owensboro Health.
Achieving excellence in healthcare is a significant achievement, and the recipient of the KAHCF/KCAL award truly exemplifies what it means to be a leader in nursing and rehabilitation services. This distinguished recognition is not just handed out to any facility; it is awarded based on rigorous quality metrics that demand performance above both state and national averages. To qualify, a facility must consistently maintain a CMS star rating of four or higher, demonstrating its unwavering commitment to providing top-quality care.
"Transitional care team members work closely with clinical education, receive dementia training, BLS training, and annual skills competency and are currently incorporating IHI Age Friendly initiatives to provide meaningful care for the geriatric population," said Hack.
