Owensboro Health Orthopedics and Sports Medicine Offering Free Sports Physicals
Owensboro Health Orthopedics and Sports Medicine is offering FREE sports physicals for middle and high school athletes. During May and June, just walk in between 8 a.m. and 4 p.m. Monday through Friday, or call 270-417-7940 to make an appointment.
Each student will need a parent or guardian present to fill out and sign paperwork before having their physical. If the paperwork is completed beforehand, they do not have to bring a parent. You may download, print and sign the forms below.
