Every child is unique. Every emergency is different. The IMAP helps doctors know what works best for your child — right away.
What is an IMAP?
An Illness Management Action Plan (IMAP) is a written guide created by your child’s specialist. An IMAP:
- Lists your child’s diagnosis and special needs.
- Explains what treatments or medicines work best.
- Notes what should be avoided.
- Gives the provider caring for your child recommendations for their care plan.

How to Get Started
- Ask your child’s specialist to complete the IMAP form.
- Email a signed copy of the form to IMAP@owensborohealth.org.
- Our team will place the form in your child’s electronic medical record.

Important: An IMAP is a guide, not a replacement for emergency medical decisions.
