From scheduling appointments to managing your medications and more, MyChart puts access to your healthcare information — and connection with your doctors — in the palm of your hand.

Create an Account

To use MyChart, you must be at least 13 years old. You can register for a MyChart account:

  • While you’re at the front desk or in the exam room during a doctor’s visit
  • With a MyChart activation code provided on your After Visit Summary
  • With self-signup online by matching your information with your medical record or with third-party identity verification
  • Downloading the MyChart app to your smartphone and following the prompts

Family (Proxy) Access

Need access to your family’s healthcare information? It’s possible to be granted proxy access.

  • MyChart Proxy Access Form: Complete this form to request access to the MyChart record of your spouse, child or another patient.
  • MyChart Proxy Revocation Form: Complete this form to terminate the proxy relationship that was previously granted. The patient chart will no longer be accessible through the proxy’s MyChart record.

Convenient MyChart Features

Request, Schedule or Cancel Appointments

Current patients can schedule an appointment with their primary care provider, request an appointment with their specialist and view past or future appointments.

Depending on the date of your next appointment, you might be able to cancel it through MyChart. If it’s too close to your appointment date or time, you’ll need to call the clinic to cancel.

Message Your Provider

Use MyChart to send secure, private non-urgent medical questions to your provider. You can use the Get Medical Advice feature to see if you should come in for an appointment, for clarification on the dosage of one of your medications, for after-visit questions or if you just want advice about a common illness.

Someone at your clinic should respond to you within two business days. If you’ve opted to receive email notifications for new messages in your MyChart account, you’ll receive a message letting you know that the clinic has responded to your request.

Manage Your Medications

See all your current medications in one place, request refills and see details like dosage, instructions, precautions and side effects.

Access, Download or Request Copies of Your or Your Family Member’s Medical Records

Save your health summary to a personal device in the case of an emergency. The portable summary includes allergies, medications, current health issues, procedures, test results and immunizations noted in MyChart. The information on your device can be uploaded into a personal health record or another healthcare organization’s electronic health record.

You can also see or print out your child’s medical record in MyChart, including immunizations.

Start a Virtual Visit

See an urgent care provider through video chat by logging into MyChart. Once you’re logged in, click “Menu” and choose “Start a Virtual Visit.” Then, select your state of residence and the reason for your visit. You will be placed in line to see the next available provider.

Check in to Appointments

Update your registration information via your MyChart account to speed up the check-in process. eCheck-in can be completed as early as five days and up to one minute before your appointment time.

Review Summaries of Your Appointments

Review your vital signs, tests or referrals that were ordered by your provider. You can also download or send your visit summary to other providers.

View Your Lab Results

View test results as soon as they become available online. You can also track results over time. For example, view all past cholesterol tests on a graph to see if you are close to meeting your target number.

Track Your Daily Health Readings Online

Your provider may request that you record information such as your daily glucose or blood pressure readings in MyChart. When you enter the information in the Track My Health feature, your provider automatically receives the readings so they can monitor your progress between visits.

Pay Your Bill

Log in to your MyChart account to access your bill and pay online. You can also use Guest Pay to make online payments for yourself or others without a MyChart account.

Let Us Know You’re On Your Way

Let us know if you’re coming to an urgent care or emergency room to be added to the waitlist. To learn more, see the On My Way frequently asked questions. For urgent care, visit the On My Way MyChart page. For an emergency visit, log in MyChart. Click “Menu” and search “On my way”. Click “Find Urgent Care on My Way”. Enable location services and choose your hospital location.

Respond to Questionnaires From Your Clinic

Your clinic might make forms available on MyChart so you can complete them online instead of filling them out in the clinic.

Consolidate Multiple MyChart Accounts

MyChart Central provides you with an access point for all your MyChart accounts with a single login, including accounts with health organizations outside of Owensboro Health. MyChart Central also provides you with Lucy, a personal health record that gives you a permanent home to organize all your medical information. Learn more about these features.

The MyChart mobile apps for iOS and Android allow you to do many of the same tasks as the MyChart website, including:

  • Viewing your test results
  • Sending and receiving messages
  • Scheduling and confirming upcoming appointments or viewing visit summaries for past visits
  • Viewing your health summary
  • Viewing preventive care procedures and due dates
  • Requesting and paying for medication refills
  • Viewing billing statements, paying balances due and visiting copays
