The heartbeat of our facility is to care for you or your family as we would our very own, and we take great care in helping with the transition from acute care to long-term care or to returning home. Whether it's to help you rehabilitate and return home to your greatest independence or to care for you in the last years, months or days of life, it's an honor to accept responsibility for you or your loved one.

Long-term Care Services

Along with intermediate and skilled nursing care with medical supervision, resident services include:

  • Dietary evaluation & assistance
  • Medication administration
  • Personal care and assistance with activities of daily living
  • Recreation
  • Rehabilitation

Short-term Care Services

Along with intermediate and skilled nursing care with medical supervision, resident services include:

  • Wound Care
  • Pain Management
  • IV Medication Management
  • Rehabilitation
  • Nutritional Support
  • Physical, Occupational and Speech Therapies
  • Support Transitioning Home

Both private or dual-occupancy rooms available, and we have a caring, committed clinical staff with an average of 15 years of experience. Our multidisciplinary team can help alleviate stress during transitions, and we encourage family or caregiver participation in all of our care planning. Our 45-bed unit is licensed by the Department of Health & Human Services in the Commonwealth of Kentucky.

We accept all insurances, self-pay, Medicaid and Medicare.
